La Angiología y Cirugía Vascular (ACV) es una especialidad tan importante como poco conocida en su preponderancia y trascendencia, al punto de asociarse (y no siempre por neófitos) a amputaciones perfectamente evitables de tocarse a tiempo las puertas de esa consulta.
Adolfo Romero Acosta, un jubilado de 61 años de edad oriundo de Baracoa, como otros tantos pacientes, lo puede atestiguar. Llegó remitido a la consulta de ACV por acusar síntomas de claudicación intermitente, es decir, se cansaba al caminar varias cuadras a cuya sintomatología agregó dolor en las extremidades inferiores durante estadios de reposo.
Acudió a su médico de familia, quien notó además ausencia de pulso periférico en las extremidades inferiores y decidió la conducta precisa: remisión a los especialistas en Angiología y Cirugía Vascular, quienes les diagnosticaron Insuficiencia arterial crónica por arterosclerosis obliterante.
“Habitualmente –precisa el Doctor Arturo Perez Chambert, responsable del servicio de ACV en el hospital general Dr. Agostinho Neto- estos pacientes son enviados a ortopedia, donde se les sigue por Artrosis con indicación de vitaminas”.
“Mientras, la enfermedad avanza silente hacia complejos estadios y cuando aparecen las lesiones o la gangrena es que finalmente el paciente llega a la consulta precisa, donde los angiólogos, por lo general, tienen que enfrentar la crucial disyuntiva entre la amputación y la muerte”, agregan los doctores Vladimir Carrión Peopes y Eurípides Cabrera Pérez, también especialistas de esa disciplina médcia.
La insuficiencia arterial crónica por arterosclerosis obliterante –explican- es la disminución del aporte sanguíneo a las piernas, o al área genital producida por obstrucción de las arterias (aorta y/o femorales), cuya causa principal está en ese taimado asesino cilíndrico relleno de picaduras de tabaco, cuya combustión llena al fumador de alquitrán, nicotina y monóxido de carbono, entre cuatro mil sustancias químicas activas que dañan los genes y de las cuales unas 43 son cancerígenas.
Romero diagnosticado a tiempo de su Insuficiencia arterial crónica por arterosclerosis obliterante fue sometido exitosamente a una intervención quirúrgica para la implantación de bypass aórtico-bifemoral por vía transperitoneal, el cual restableció el flujo sanguíneo hacia las extremidades inferiores, conjuró la amenaza de amputación y le garantiza la existencia en condiciones normales.
Valorada en más de 80 mil dólares en el mundo exterior, la operación es gratuita en Cuba y exige, además de la importación de la prótesis vascular y los materiales quirúrgicos, de una alta especialización quirúrgica por lo complejo de la intervención.
Cuatro cirujanos participan simultáneamente en la implantación del bypass aórtico-bifemoral, técnica quirúrgica, cuya realización en la provincia de Guantánamo cobra particular relevancia por garantizarse con recursos mínimos y evitar que los pacientes locales dependan de las disponibilidades de las instituciones foráneas en condiciones de practicarla, principalmente en la lejana Habana.
“El servicio de Angiología y Cirugía Vascular del hospital general Dr. Agostinho Neto cuenta con cinco especialistas, cuatro de los cuales asumen el proceder quirúrgico resolutivo de una patología diagnosticada con poca frecuencia, pero padecida por muchas personas a partir de los 45 años de edad por no seguir patrones de conducta saludables”, explica Pérez Chambert.
Las estadísticas de Salud en la provincia de Guantánamo reportan casi un centenar de amputaciones anuales en el servicio de ACV, el 80 por ciento de las cuales la sufren pacientes descuidados de su diabetes mellitus y el resto portadores de arterosclerosis obliterante atendida tardíamente.
“A la reducción de esas estadísticas, y sus efectos negativos física y psíquicamente en el paciente amputado, y la economía nacional por disminución de la fuerza laboral activa e incremento de la seguridad social, puede contribuir este servicio”, añade el especialista de segundo grado en angiología y cirugía vascular.
Lesiones cerebrales por embolismo, dependientes en más del 90 por ciento de los casos de la bifurcación de las arterias carótidas; trombosis mesentéricas, hipertensión renovascular e impotencia sexual, muchas de ellas de dolorosos desenlaces, pueden prevenirse y evitarse con la intervención temprana de esta especialidad.
-La mejor medicina es la que precave-, dijo con el acostumbrado acierto José Martí, comentó Pérez Chambert, y aseguró que continuar la preparación angiólogos-cirujanos vasculares en Guantánamo, garantiza el crecimiento cualitativo del servicio en esta especialidad, practicada desde los tiempos de Hipócrates y Galeno.

When Romero Acosta visited his family doctor the physician noticed an absence of outlying pulse in the lower limbs and right away he decided to transfer him to the Vascular Angioloy Surgery specialists who diagnosed him a chronic arterial insufficiency caused by an obliterating arterosclerosis.Dr Arturo Pérez Chambert, head of the Vascular Angioloy Surgery service in the Dr Agostinho Neto General Teaching Hospital of Guantánamo explains that these patients are frequently sent to the orthopedics, where they are follow up for the condition of arthrosis with a prescription of daily vitamins.
"Meanwhile, the illness is silently progressing toward complex stages and when skin lesions appear or gangrene in the worst cases is the patient visits the right physicians, the angiolgists, in general, have to face the crucial alternative between amputation and death", added Vladimir Carrión Peopes and Eurípides Cabrera Pérez, who are specialists of this medical discipline.
“Chronic arterial insufficiency caused by obliterating artherosclerosis-explain the specialists- is the decreasement of blood flow to the legs, or to the genital area caused by an obstruction of the arteries (the aorta or femoral). The main factor that causes this condition is the nicotine consumption by smokers along with the carbon monoxide, among other 4,000 chemical active substances that damage the human genes, 43 out of them are carcinogenic”.Romero, who was diagnosed on time of his condition was successfully subjected to a surgical intervention with a transperitoneal aortic-bifemoral bypass implantation which reestablished the blood flow toward his lower limbs, preventing this way an amputation, saving and guaranteing his ilfe on normal conditions.
The complex surgical procedure is internationally valued in more than 80, 000 US dollars, but in Cuba this operation is free.
To perform this procedure it is necessary the import from the world market the vascular prosthesis, as well as the surgical materials. The operation includes a high tech specialization of the Cuban surgeons because of the complexity of the surgical.
Four Guantanamo surgeons simultaneously take part in the implantation of the aortic-bifemoral bypass, a surgical technique performed in the province of Guantánamo which has a particular relevance because is guaranteed with minimum resources avoiding the local patients to depend on the availabilty of far away national health institutions, mainly in the distant Havana city."The Vascular Angiology Surgery service of the Guantánamo General Hospital has a five- specialist team, four of those surgeons perform the surgical procedure of a pathology which is not diagnosed frequently, but it is suffered by many people from 45 years old on who do not follow a healthy life style", explains Pérez Chambert.
The health statistics of Guantánamo reports a hundred annual amputations in the VAS service, 80 percent of them are performed to careless patients suffering diabetes mellitus, the rest of them bear obliterating arterosclerosis assisted belatedly.
"The VAS service of Guantanamo contributes to the reduction of those statistics, the negative physical effects in amputated patients, and the decrease of the national labor active force, increasing thus the costs of the social security”, added the second degree specialist in vascular angiology surgery.Several conditions with painful outcomes can be prevented with the performance of this high tech surgical procedure such as mesenteric thrombosis, renovascular hypertension, sexual impotence and cerebral injuries caused by embolism, more than 90 percent of these cases depend on the bifurcation of the carotid arteries.
